At Forte Health Medical Clinic, we practice primary care in a way that makes sense to us.
We’re a direct primary care clinic, which may work a bit differently than what you’re accustomed to. Our patients pay a flat monthly fee for care in our personalized practice:
Clinical visits without an additional copay or surprise bill
Extended appointments to adequately address health concerns
Care focused on the patient instead of on billing insurance
Same-day or next-day appointments typically available
Options for phone, video, and home visits
Exclusive direct communication with your doctor, not a call center

Why membership?
Having a reliable, trustworthy physician in your corner is invaluable, especially in times of need. The quest to find one can be arduous and require monk-like patience. So naturally, once you’ve found them you’d want to keep them, right? Well if you’ve ever lost access to “that one great doctor” due to an employer or employment change, you know that’s not always your decision to make. It’s disruptive and sad on both sides, as doctors are also disheartened to see great patient-physician relationships come to an abrupt end.
A membership allows us to continue caring for you through life’s inevitable changes. It promotes uninterrupted relationship-building, which is a cornerstone for personalized healthcare that aligns with your goals.

For new patients, a one-time nonrefundable enrollment fee of $100 per person applies.
For returning patients, a re-enrollment fee of $250 per person applies.
*Minors (17 and under) must have an accompanying adult membership age 19+.
**Waitlist only - Currently we are accepting Medicare patients onto a waitlist only as we have not yet opted out of Medicare.
How do I Join?
Step 1. Go through the FAQs and our site to get most of your questions answered.
Step 2. Schedule your complimentary virtual meet & greet with Dr. Siu to see if we’re a good fit for you.
Step 3. Invest in your health by joining Forte Health. Once your file is reviewed, you’ll receive a confirmation email.
Step 4. Schedule your first 1-hour virtual appointment with Dr. Siu to establish care.
Step 5. We’ll request any necessary records from your previous doctors to prepare for your first visit.
Step 6. Enjoy the process as we start our journey together to your good health.
